After you have opened a map, enabled the ArcGIS Publisher extension and added the Publisher toolbar to ArcMap, you simply have to click the Publish Map button (or click Publisher and click Publish Map) to publish the map. You will be prompted to browse to a location to save the Published Map Document. The default PMF file name will be the same as the map document file name, though you can change it.
